Tibialis Posterior Tendonitis... Where To Find The Latest Treatment Information.

WHAT IS IT?


The tibialis posterior muscle starts at the back of the leg beneath the major calf muscles. It then runs along the inside of the leg behind the inside (medial) aspect of the ankle and inserts into the medial aspect of the foot in the region of the navicular an d cuneiform bones. The tibialis posterior muscle assists in supporting the medial longitudinal arch of the foot and it also helps to plantarflex the foot.

If the tibialis posterior muscle undergoes excessive contraction forces, damage or overuse of the tendon can occur and lead to inflammation and eventually dysfunction and degenaeration of the posterior tibial tendon.

WHAT CAUSES IT?

Factors which may contribute to the cause of tibialis posterior tendinitis include:

Poor foot posture
Unsuitable footwear
Training errors
Insufficient stretching
Muscle weakness

HOW IS IT TREATED?

1. Decrease aggravating activities. If pain resumes, back off.

2. Decrease length of stride and intensity of running

3. Ice after activity only (NEVER ICE before activity or first thing in the morning)

4. Gently stretch calf muscles before jumping out of bed.

5. Shoe inserts may be needed

6. Footwear needs to be reviewed

7. Massage often helps

8. Strengthening exercises may need to be prescribed.

9. Orthotic therapy is often needed when the foot hyperpronates. Special modifications such as medial heel skives in orthotics may be needed. This can only be performed following an appropriate laser scan or plaster cast of the foot.

10. Extracorporeal Shock Wave Therapy or ESWT is a newer treatment modality with promising results.
